Texas Christian Wellness is committed to protecting the privacy of your health information. This Notice of Privacy Practices explains how we may use and disclose your protected health information (PHI), your rights regarding your PHI, and our legal duties with respect to your PHI.

We may use and disclose your PHI for activities necessary to run our practice, such as quality assessment, training, and business management.

Other permitted uses and disclosures

• For health oversight activities, including audits, investigations, and licensure proceedings.

Uses and disclosures requiring your authorization

For any use or disclosure of your PHI other than those described above, we must obtain your written authorization. You may revoke an authorization at any time in writing, except to the extent we have already relied on it.

Your rights regarding your PHI

You have the following rights regarding your PHI:

• Right to inspect and copy — You may request to inspect and obtain a copy of your PHI.
• Right to request amendment — You may ask us to amend your PHI if you believe it is incorrect or incomplete.
• Right to an accounting of disclosures — You may request a list of certain disclosures we have made of your PHI.
• Right to request restrictions — You may ask us to restrict the way we use or disclose your PHI. We are not required to agree to all restrictions.
• Right to confidential communications — You may request that we communicate with you in a certain way or at a certain location.
• Right to a paper copy of this Notice — You may request a paper copy of this Notice at any time.

If you are a current or prospective client, please do not include protected health information in any website form, email, or text message. For treatment-related communication, use the secure client portal or call our office directly.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
